IV Therapy Annual Report

IV Therapy Providers are required to submit to the board an annual report for the period of July 1 through June 30 of the respective year (K.A.R. 60-16-103).

Please review these instructions before completing the annual report:

1.      The IV Therapy annual report is due no later than July 31 and covers data from July 1 of the prior year through June 30 of the present year. All data must be submitted for each course given during this time period. There is no fee to be submitted with this annual report. This report will be submitted electronically and there are no attachments you need to submit with this annual report.

2.       Single program continuing nursing education (CNE) providership: Effective for two years and may be renewed by submitting the single offering CNE provider application and by paying $100.00. Each single program CNE provider who chooses not to renew the providership must notify the board in writing of the location at which the rosters and course materials will be accessible to the board for three years. Two year renewal notices will be emailed to providers with deadlines for the submission of their renewal application.

3.       Long term CNE provider: Each long-term CNE provider shall submit the materials outlined in 60-16-103 (b) with the five-year long-term provider renewal. Five year renewal notices will be emailed to providers with deadlines for the submission of their renewal application. There is no fee for the five-year renewal.
